Residents we're typically a good fit for

Where another setting is safer
- Ongoing skilled-nursing-level needs, ventilator support, or complex wound care requiring 24-hour licensed nursing presence
- Acute medical instability that has not resolved
- Exit-seeking or elopement risk requiring a secured memory care unit
- We say this early rather than late. If we're not the right setting, we'll say so on the first call and point you somewhere honest.
What we need to say yes
A current picture of care needs
Assistance level, mobility, cognition, continence, behaviors, and any two-person transfer needs. A face sheet and current medication list cover most of it.
Recent clinical documentation
History and physical, recent progress notes, and the discharge summary when it's available. We'll also need a current physician assessment before move-in.
A named family decision-maker
Whoever holds power of attorney or is leading the decision. We work with the family directly from there.
An honest note on acuity
Higher-acuity needs are assessed at intake and quoted from the start. Our RN-MSN clinical standard and trained team let us support more than most homes this size — but we'd rather decline than over-promise.

Nurse-led standards, small-home care
An RN-MSN in the owning family sets the clinical standard. An LPN manager and a CNA operations manager run the daily rhythm on site.
One all-inclusive quoted rate
Care, meals, laundry, housekeeping, and daily support in one rate quoted after assessment. Care-level reviews are discussed with the family before anything changes.
Aging in place
Residents can stay as needs increase, with hospice partners coming to the home. The goal is that no one has to move again.
