Rehabilitation may end. Support does not have to.
Friendship Village Tempe provides short-term rehabilitation and skilled support. Sweetwater Groves does not replace that work. It offers an appropriate next setting when intensive rehabilitation has ended but daily assistance, medication support, watchful attention, and a long-term home still matter.

An RN-MSN in the owning family sets the clinical standard and trains the team. An LPN manager and CNA operations manager lead daily work on site.

Movement already included
Living Gracefully's regular strength, balance, and mobility program is included with the stay, with no separate Sweetwater Groves fee.
Visiting professionals welcomed
When appropriate, residents may continue with qualified PT, OT, speech, or home-health providers. We coordinate the daily follow-through between visits.
What should come with your loved one
A clear handoff helps everyone begin from the same understanding. Before leaving Friendship Village Tempe, ask for:
- The current medication list and the time each medication was last given.
- Mobility, transfer, fall-prevention, and equipment guidance.
- Therapy recommendations and instructions for safe practice between visits.
- Follow-up appointments, transportation needs, and the names of continuing providers.
- Nutrition or swallowing guidance when it applies.
