    Shea Post Acute Rehabilitation Center provides post-acute and skilled rehabilitation. Sweetwater Groves begins where that chapter may end: a residential home for an appropriate loved one who needs daily personal support, careful routines, and continued coordination with visiting professionals.

    An RN-MSN in the owning family sets the clinical standard and trains the team. An LPN manager and CNA operations manager lead daily work on site.

    Ten private rooms, familiar caregivers, and a quiet daily rhythm give recovery room to become confidence and belonging.

    Movement already included

    Living Gracefully's regular strength, balance, and mobility program is included with the stay, with no separate Sweetwater Groves fee.

    Visiting professionals welcomed

    When appropriate, residents may continue with qualified PT, OT, speech, or home-health providers. We coordinate the daily follow-through between visits.

    What should come with your loved one

    A clear handoff helps everyone begin from the same understanding. Before leaving Shea Post Acute Rehabilitation Center, ask for:

    • The current medication list and the time each medication was last given.
    • Mobility, transfer, fall-prevention, and equipment guidance.
    • Therapy recommendations and instructions for safe practice between visits.
    • Follow-up appointments, transportation needs, and the names of continuing providers.
    • Nutrition or swallowing guidance when it applies.

    Both homes are in the 85260 area, which may make it easier to preserve familiar family visits and local care relationships. Sweetwater Groves never assumes that geography alone makes the fit; the current discharge picture guides every decision.
